Database / Show form Data on content page

Hi, i'm so desperate to show data that i've collected with form on my content page on the frontend. I've purchased two extension (rsform Pro & Table manager) but i can't get the desired results. I'll appreciate your help. I'm not good in programming. All i need is to show Three fields.

I've done so but the photos flicker where the overlap.

Hi,
   I'm just a novice with oziogallery. I've done some installation 
assistance i found in this forum but i could not show photos on my
gallery. I tried online and on localhost but no ways. Here's my test site: http://students.nucssa.net/test/images/oziogallery/gallery.php
I also created a Gallery item on my Top Menu. You can have a look.
Ps:I copied my photos into the test test/images/oziogallery folder. What can be the problem? I'll appreciate anyone's help.
